I just tried the plugin and it wouldn't provide a settings screen. It just kept saying "install Disqus."
I figured it has something to do with MT5 being a later version.

This plugin isn't compatible with MT5. There's not a lot of info on this--I found out by contacting Disqus support directly. They say they're updating the plugin and should have a working version soon, but I wouldn't hold my breath.
If you want, you can submit a ticket (like I have) and maybe if people make enough noise they'll hurry up a little more: http://disqus.zendesk.com/requests/portal/new
In the meantime you can probably use the manual installation for Disqus--not as convenient as the plugin, but it works.
I've submitted a request for this as well, and they told me they are working on it, but no new news. Please, everyone who uses Disqus on MT, submit a request at Disqus' KB for this:

Same story here... I can't even manually implement Disqus into the templates because there's no apparent way to import MT comments into Disqus, as the import screen instructs you to use the plugin...
