How to make an e-mail notification

A Web form with Email notification has two parts to it--the HTML portion and the PHP code. The HTML portion is used to create the form. Each element of the form has a "name" attribute which is used to send the user input to the PHP code. The PHP code is used to process the user input and place it into an email which is sent to the webmaster.
