Documentation for dynamic publishing?

Since searching this forum and the MT documentation times out (!!), can someone please point me to documentation for dynamic publishing? The only page I'm able to find is:

or outdated pages like:

I'm using MT 4.32, but a full rebuild is not altering the site's .htaccess file to redirect incoming URL requests. All of the previously static files are now named myfile.php.static. After rebuilding, links to all of the previously static URLs are broken.

Alternatively, how does the .htaccess file need to be updated to handle incoming URL requests?

Also, do the MT templates need to use different tags? How do these tags interact with PHP given that dynamic publishing uses PHP to begin with?

So many questions, so few answers to be found...
