There is this plugin http://www.majordojo.com/projects/movable-type/contact-forms/, which is on my 'to try' list, still in beta at the moment.

I am doing that for over 4 years now!
The way I did this was to use a 'blog' only for this purpose.
Basically I've used the comments facility as the form mechanism.
I've changed the comment form field names to what I wanted and so people were filling that form and submitting it.
The comments were set to be held for approval before publication so that I could just get an email notification with the form results, but not display them online.
This is pretty much it!
