You tried to set inexistent column origname to value Commenter Registration on MT::Template at /usr/home/joschino/publichtml/cgi-bin/managed-mt/plugins/CommentRegistration/CommentRegistration.pl line 187

The other problem is, I can’t figure out how to restore the default! Please help!

If any modifications happened to your main sign-in templates, you should be able to fix that by refreshing the contents of the tmpl directory under the main MT directory. Usual caveat: backup first.
