I'm trying to change my blog's template set, on an installation recently upgraded from 4.1 to 4.21 Pro by completely wiping and reinstalling my mt directory.
Refreshing blog templates causes an error in the Activity Log along these lines:
Notepad-Chaos Template Set died with: Undefined subroutine &MT::Plugin::NotepadChaosTemplateSet::ontemplateset_change called at lib/MT/Callback.pm line 35.
(and a similar error for Mid-Century)
Also, the dialog box for refreshing template sets doesn't list Mid-Century, despite that plugin being installed. New blogs can't choose that set either. Attempting to set templates to Sandbox throws the same errors - that is, exceptions thrown for Notepad-Chaos and Mid-Century.
I've tried removing non-core plugins and reinstalling, but this issue persists.
I'm stuck. Any thoughts?

Try removing the Notepad-Chaos Template Set. I had a similar error in the Activity Log.
Toni's right. The template set plugins control this behavior. If you're having problems with them loading, chances are the plugin isn't working properly with 4.21.
Big thanks - I was heading towards the same conclusion, as nuking all my templates and starting over with Classic Blog still threw the `on_template_set_change` error.
I hauled out both Mid-Century and Notepad-Chaos plugins, refreshed templates back to Classic Blog, then reinstalled just Mid-Century. Things seem to be more-or-less working now.
